Which patients can be referred?
All type 1 and 2 diabetes patients (including patients with complications) requiring specialist intervention.
Which patients cannot be referred?
Medical emergencies
Diabetes in pregnancy/children/adolescents
Type 1 diabetes with advanced complications
Advanced renal disease requiring dialysis
Why should patients be referred?
Patient care will be based on best practice evidence, reflecting NICE and Map of Medicine guidance. Patients can expect shorter waiting times, a greater choice of appointment dates and times, and an exceptional quality clinical service.
Referring your patient
Referrals can be made either through the paper referral form, downloadable here or via Choose and Book. Paper referrals can be sent to the Administration Office by fax, email or post.

Triage Process
All referrals received are read and classified within two to four working days of receipt. Any referrals that do not meet the agreed service criteria or have incomplete clinical details would be returned to referring clinician within one working day of triage. Triage is carried out by a specialist clinician (GPSI) who is skilled and accredited to deal with the level of clinical decision involved for the service.
Appointments
All patients considered for treatment within the service would have their appointments confirmed within 48-72 working hours from the date of the referral being triaged and accepted within the service. We aim to provide patients with an appointment within four weeks.
For paper referrals, patients will receive an appointment letter in the post soon after their referral is accepted, and can contact the administration office if they wish to change their appointment.
Clinic Letters
A comprehensive clinic letter outlining the diagnosis, investigations, management plan and patient advice following each patient consultation will be sent to the referring clinician within five to seven working days of consultation.
